The Department of Health (England) has published a booklet that has been developed for social care workers to support the integration of Your health, your way ? a guide to long term conditions and self care into practice. It has been circulated to healthcare professionals, including GPs and each section of the booklet has been developed to raise awareness of the different aspects of the guide, which was launched on NHS Choices in November 2008.
